Gender Identification and your GP Record

As a practice, we welcome patients speaking to us about any concerns or decisions made regarding their gender identification. Following recent national guidance, we have updated our processes in order to best support our patients. One aspect is for us to consider how we best manage patients’ GP records. We value individual patients’ requests and ideas for how we do this so that important decisions can be made together.

Any request we receive from a patient to change their name, gender marker or title on their medical record is accepted by Harewood Medical Practice. We require no Gender Recognition Certificate or updated birth certificate to proceed. All you need to do is to let us know.
